Serologic testing for celiac disease is recommended for symptomatic individuals and specific at-risk groups. All testing performed to diagnose celiac disease must occur while the patient is consuming a gluten-containing diet.

Initial testing for celiac disease includes an assessment of immunoglobulin A (IgA) and tissue transglutaminase (tTG) IgA antibody concentrations. Additional antibody testing may be indicated depending on initial results.

Recommendation

The preferred reflex test for suspected celiac disease is Celiac Disease Reflexive Cascade, Serum (3016817). Individual antibody tests may also be considered.

Total IgA concentrations can be assessed using Immunoglobulin A (0050340). In patients with sufficient IgA, the preferred single test for celiac disease is Tissue Transglutaminase Antibody, IgA (3016860). In patients with selective IgA deficiency, Tissue Transglutaminase Antibody, IgG (0056009) is the recommended initial test.
